Tong Mai Si Ni Tang (通脈四逆湯) is Si Ni Tang with increased Fu Zi and Gan Jiang; it strongly rescues collapsed yang and unblocks the pulse, for severe yang collapse with internal cold and a false-heat appearance (yin repelling yang).

Indications
Severe yang collapse with internal cold and external false heat: cold limbs and severe interior cold with diarrhoea of undigested food, yet a flushed face, and a faint or imperceptible pulse.
Tongue & Pulse
Tongue: pale with a white coat. Pulse: faint, almost imperceptible.
Modifications
For severe collapse; add Ren Shen if qi is also exhausted.
Cautions & Contraindications
Emergency formula for true cold with false heat; never for true heat. Use prepared Fu Zi.
