Network Meta-Analysis Finds Heat-Clearing Traditional Chinese Medicine Injections May Improve Outcomes in Viral Pneumonia When Added to Conventional Treatment

Viral pneumonia remains a significant global health burden, and traditional Chinese medicine has a long history of addressing respiratory infections. A class of traditional Chinese medicine injections derived from classical formulas — those with so-called “heat-clearing and detoxifying” properties — has been widely used alongside conventional medicine in clinical settings, particularly in China. However, comparative evidence on which specific injection performs best has been limited.

This systematic review and Bayesian network meta-analysis pooled data from 83 randomised controlled trials involving 8,678 participants. Researchers searched eight databases for studies published up to December 2025 that tested seven different heat-clearing traditional Chinese medicine injections combined with conventional medicine against conventional medicine alone or each other. The injections examined included Reduning, Xiyanping, Yanhuning, Tanreqing, Xuebijing, Shuanghuanglian, and Qingkailing formulations.

Across multiple clinical endpoints, adding traditional Chinese medicine injections to conventional medicine generally outperformed conventional medicine alone. Reduning injection combined with conventional medicine showed the most consistent benefits — shortening cough duration and hospital stay, reducing the inflammatory marker interleukin-6, improving immune markers, and notably also showing fewer adverse events than conventional medicine alone. Qingkailing combination performed best for overall clinical response rate, fever reduction, and lung rales, while Yanhuning combination was most effective for breathing difficulties and certain inflammatory markers.

Conclusion: This analysis suggests that heat-clearing traditional Chinese medicine injections, particularly Reduning, may offer meaningful clinical benefits as an adjunct to conventional treatment for viral pneumonia, though the findings should be interpreted cautiously given variability in trial quality and the predominantly Chinese study populations.
